Disclaimer

Grade is the most severe feature present. A vascular injury, defined by the 2018 update as a pseudoaneurysm or arteriovenous fistula, or active bleeding raises the grade on its own: contained within the organ that is grade III for liver and kidney, and grade IV for spleen. Advance one grade for multiple (bilateral, for kidney) injuries, up to grade III. These are the CT criteria; the AAST grade is the HIGHEST of the imaging, operative and pathologic criteria, so operation or pathology can raise it. Management depends on haemodynamic stability, not the CT grade alone.
