Lung-RADS 2022 (Baseline)
ACR Lung-RADS v2022 category for a baseline low-dose CT screening exam. For part-solid nodules the solid-component size drives the category. (Growth-based categories on follow-up are not modelled here.)

Formula
Solid: <6 → 2, 6–<8 → 3, 8–<15 → 4A, ≥15 → 4B. Part-solid ≥6 mm: solid component <6 → 3, 6–<8 → 4A, ≥8 → 4B. Ground-glass: <30 → 2, ≥30 → 3.
Disclaimer
For lung-cancer screening LDCT only. This models the baseline category from size alone; category 4X (additional suspicious features) and growth-based categories on follow-up are not computed. A category 1 requires no nodules or benign calcification/fat. Size is not the only route into a category, and the v2022 table has entries this cannot see: a JUXTAPLEURAL nodule that is solid, <10 mm, smoothly marginated and oval, lentiform or triangular is category 2 whatever its size says here; atypical pulmonary cysts (3/4A/4B) and airway nodules (2/4A/4B) are categorised by morphology and location, not diameter. NO MALIGNANCY PROBABILITY IS GIVEN: the v2022 chart does not state one. The percentages this calculator used to print were the v1.0/v1.1 “Probability of Malignancy” column, carried forward, and a v1.1 number shown under a v2022 heading reads as current when it is not.
