Modified CT Severity Index (mCTSI)
Modified CT Severity Index (Mortele 2004) for acute pancreatitis. It simplifies the peripancreatic inflammation grade, scores necrosis in two tiers, and adds extrapancreatic complications, correlating with outcome at least as well as the classic Balthazar CTSI and more reproducibly.
One or more of: pleural effusion, ascites, vascular, parenchymal, or GI-tract involvement.

0 / 10
Formula
mCTSI = inflammation (0/2/4) + necrosis (0/2/4) + extrapancreatic complications (0/2). Mild 0–2, moderate 4–6, severe 8–10.

The modified (Mortele) CTSI; the classic Balthazar CTSI (grade A–E + necrosis, 0–10) is an alternative. Contrast-enhanced CT for necrosis is best assessed after ~72 hours. Correlate with clinical severity scores.
