PI-RADS v2.1

PI-RADS v2.1 assessment category for a single lesion. The dominant sequence depends on zone: DWI drives the peripheral zone, T2 drives the transition zone. DCE upgrades a peripheral-zone DWI-3, and DWI can upgrade a transition-zone T2-3.

PI-RADS v2.1 criteria. The 4/5 split is SIZE: a focal lesion otherwise scoring 4 becomes 5 at ≥1.5 cm in greatest dimension, or with definite extraprostatic extension. This calculator takes the score you have already assigned and does not derive it.

PI-RADS v2.1 criteria. In the transition zone a fully encapsulated nodule scores T2W 1 (made explicit in v2.1). This calculator takes the score you have already assigned and does not derive it.

PI-RADS 3

Equivocal. Consider PSA density / targeted biopsy.

PI-RADS 3: Intermediate
Formula

PZ: DWI is dominant (DCE upgrades DWI-3 → 4). TZ: T2 is dominant (T2-2 with DWI≥4 → 3; T2-3 with DWI-5 → 4). The DWI 4/5 split is ≥1.5 cm or definite extraprostatic extension.

Per-lesion assessment requiring adequate multiparametric MRI. DCE affects only the peripheral zone. Not validated after treatment or for the central zone / anterior fibromuscular stroma. Correlate with PSA density and clinical context.
